All registered participants must comply with all of the Sun Valley Lacrosse COVID-19 Safety Protocols. As a registered participant you are confirming that you have read and are aware of all Sun Valley Lacrosse COVID-19 Safety Protocols, and are agreeing to comply.

You are aware of the risks of COVI0-19 and participating in Sun Valley Lacrosse programs. While Sun Valley Lacrosse has put in place protocols that will be implemented as preventative measures to reduce the spread of COVID-19, Sun Valley Lacrosse cannot guarantee that you or your child(ren) will not become infected with COVID-19. Furthermore, attending Sun Valley Lacrosse programs could increase your risk and your child(ren)'s risk of contracting COVID-19.

If a person has had known exposure to COVID-19, or is experiencing any COVID-19 symptoms you agree that neither the player nor parent will attend Sun Valley Lacrosse programs. If a player is unable or unwilling to follow the Protocols, you are aware that staff may dismiss the player from that and future practice sessions.

As a registered participant, you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that your child(ren) and you may be exposed to or infected by COVID-19 by attending Sun Valley Lacrosse programs and that such exposure or infection may result in personal injury, illness, permanent disability, and death (collectively, "Injury"). You understand that the risk of becoming exposed to or infected by COVID-19 at Sun Valley Lacrosse programs may result from the actions, omissions, or negligence of myself and others, including, but not limited to, staff, volunteers, and program participants and their families. You voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to your chid(ren), damage, loss, claim, liability, or expense of any kind, that you or your child(ren) may experience or incur in connection with your child(ren)'s attendance at or participation in Sun Valley Lacrosse programming ("Claims"). On your behalf, and on behalf of your children, you hereby release and hold harmless Sun Valley Lacrosse, its coaches, volunteers, employees, agents, officers and directors, of and from the Claims, including all liabilities, claims, damages, costs or expenses of any kind arising out of or relating thereto. You understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Sun Valley Lacrosse, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Sun Valley Lacrosse program. As a registered participant, you are confirming that you have read, understand and agree to comply with the Covid-19 Safety Protocols and Guidelines.
